Flu Shots, October 20, 2021
The Department of Health is offering flu vaccinations at the Timber Lake School on Wednesday, October 20th. A child’s private health insurance or Medicaid will be billed for the cost of the vaccination. Children without health insurance or Medicaid may receive the flu vaccine at the school through the Vaccines for Children program. The Vaccine Information Statement and consent form are included for your convenience. The consent form must be completed, signed, and returned to the school along with a copy of the child’s insurance or Medicaid card by Thursday, October 14th. Students without a completed consent form and a copy of the insurance or Medicaid card will not be vaccinated. Staff may be vaccinated if they provide a copy of their insurance or Medicaid card by October 14th or they may pay $46 for the vaccine.
